Care and Services
What we help with, and how we decide together what each person needs.
Every resident starts with an assessment and a care plan written with them and their family. The list below is what the home is set up to provide; what each person receives is set out in their own plan and reviewed as their needs change.
- Personal CareHelp with bathing, dressing, grooming and getting ready for the day, at the resident's own pace and with as much independence as they want to keep.
- Mobility and TransfersStandby help when walking, a hand with transfers, and support with walkers and wheelchairs. The house is on one level with a ramp at the front door.
- Toileting and Continence CareDiscreet, regular support with toileting and continence, planned around each person's routines and privacy.
- Meals and NutritionThree meals and two snacks a day, cooked in the home. Soft, diabetic and low-salt meals are prepared when the care plan calls for them.
- Medication SupportReminders and help with medicines as set out in the care plan, with records kept for every dose.
- Nurse-Delegated CareTasks such as insulin or wound care can be delegated by a registered nurse to trained caregivers for a specific resident. Sarah oversees delegation herself.
- Activities and Social SupportGardening, music, cards, short walks and outings, planned around what each resident enjoys and is able to do on a given day.
Who Juniper Lane may suit
Who fits well
- Someone living with dementia who does best with a steady routine and familiar faces
- Someone who needs help with bathing, dressing and walking but wants to keep doing what they can
- Someone more at ease in Spanish or Tagalog
- Someone who would enjoy a garden, a shared table and a small household
Where we cannot help
- Needs that call for a nurse in the house around the clock
- Behaviour that puts the other five residents at risk
Dementia specialty
The home holds a Dementia specialty designation. Caregivers have completed the state's dementia training, and routines are kept steady for residents who find change hard.
Mental Health specialty
The home holds a Mental Health specialty designation and works with each resident's outside providers on their plan.
What we talk through before a move
Before anyone moves in, we go over the care plan, the room, the costs and anything the home cannot do. Some needs are better met elsewhere, and we will say so.
